Telehealth Informed Consent
Please review this information before your first telehealth visit with AMMG Medicine.
What is telehealth?
Telehealth is the delivery of health care services using secure, real-time audio and video technology when you and your provider are in different locations. Telehealth visits may include evaluation, diagnosis, consultation, treatment, medication management, and education.
Benefits
- Convenient access to care without travel
- Less time away from work, school, or family
- Easier follow-up and continuity of care
Possible risks and limitations
- Technology problems, such as a poor connection, may interrupt or delay a visit.
- Your provider cannot perform a full physical exam and may determine that an in-person visit is needed.
- Although we use secure platforms, no electronic system is completely free of privacy and security risks.
Privacy and security
We use telehealth platforms designed to protect your privacy and comply with HIPAA. Visits are not recorded without your consent. Please join from a private location, and let your provider know if anyone else is present.
Location and licensing
You must be physically located in Texas, or another state where your provider is licensed, at the time of your visit. You will be asked to confirm your location at the start of each visit.
Emergencies
Telehealth is not for emergencies. If an emergency occurs during a visit, your provider may contact emergency services on your behalf, so please share your current address at the start of each visit. If you are disconnected, your provider will try to reach you at the phone number we have on file.
Prescriptions
Prescriptions, including any controlled substances, are issued only when clinically appropriate and in accordance with federal and Texas law. Some medications may require an in-person visit.
Billing
AMMG is a self-pay practice. Telehealth visits are priced like in-person visits, and payment is due at the time of service. You will receive a Good Faith Estimate before your visit.
Your rights
Participation in telehealth is voluntary. You may decline or stop a telehealth visit at any time and request in-person care instead, without affecting your right to future care.
Consent
By scheduling and participating in a telehealth visit, you acknowledge that you have read and understand this information, have had the opportunity to ask questions, and consent to receive care through telehealth. Our staff may also ask you to confirm your consent verbally or in writing.
